1. Select the Application / Enrollment Agreement for the course and student status that applies to you from the Enrollment Agreement menu at the right.


2. Each Enrollment Agreement consists of four pages. Print out all four pages of the agreement by clicking on the "Printer Icon" on the tool bar of the Adobe Acrobat page.


3. Read the entire Application and Agreement carefully.


4. If you wish to enroll, fill in all of the information requested and sign the agreement on page 3 where indicated. Enclose all four pages of the agreement and mail the completed agreement and your payment to the Admissions Office at the address on the application.


Note: U.S. student applies to anyone residing in the United States of America or any of its territories, and the Commonwealth of Puerto Rico. It also applies to U.S. military service persons or U.S. government employees wherever they are located. All other applicants are considered International Students.
